You are to assume that you are working as a Tax Manager, working for a firm of accountants.

A new client, Nick White, has asked your firm to act as tax advisers to both himself and his wife, Caroline.

Nick is self-employed and his wife is employed as his assistant. She answers the telephone, makes appointments, produces invoices and chases late paying clients. She would also expect to help out in times of emergency, in whatever capacity she can, to ensure that deadlines are met.

His taxable profits have been steadily rising over the last few years, but he expects that they have now stabilised at £115,000 per annum. This is before paying £5,000 salary to Caroline.

Nick has asked for advice on whether to incorporate his business. In particular, he requires details of:
1.    the general advantages and disadvantages, given his level of profit.
2.    the advantages and disadvantages of setting the company up with equal shareholdings with his wife. She would be registered as company secretary.
3.    tax-efficient profit withdrawal. He wishes to leave £30,000 per year in the business to ensure he can afford up to date equipment, for emergencies and expansion if necessary.

Nick has asked for a meeting to discuss the matter, but in the meantime would like a preliminary report setting out the general factors to be taken into account.

Required:
1.    to prepare a report for Nick, answering (generally) his queries as above.
2.    to also include within the report details of any additional information, together with reasons, that you require before you would be able to finally advise him as to a prospective company’s shareholding and efficient profit extraction.

MAP-IT means: Mobilize, Assess, Plan, Implement, and Track

To begin to achieve the goal of improving health, a community must develop a strategy. That strategy, to be successful, must be supported by many individuals who are working together. 

In much the same way you might map out a trip to a new place, you can use the MAP-IT technique to ‘map out’ the path toward the change you want to see in your community.

For the Session Long Project (SLP) of this course, you will follow one of the following families:

    1) A non-English speaking immigrant family.  The parents, (35 year old father and a 31 year old mother) have a one-year old son. They (the parents) are obese, smokers, alcoholics and have only a high school education.  Due to their weight, the mother has diabetes and high blood pressure, and the father has high blood pressure and kidney disease. The parents are a low income family with no insurance.  You will guide the parents on how to achieve the minimum state health requirements for both the child and themselves, i.e. vaccination, good nutrition, a safe and healthy home environment etc.; or,

    2) An English-speaking American family.  The parents, (42 year old father and a 40 year old mother) have two children, a 14-year old son and a 18 year old daughter. They (the parents) are obese, smokers, and have a history of heavy drug use in their late teen and early adult years.  The father has only a high school education while the mother has a BA in English Literature.  Due to their weight, the mother has diabetes and high blood pressure, and the father has high blood pressure and liver aliments. The son suffers from asthma while the daughter has had two previous abortions. She may be drug dependent. The parents are a moderate-low income family with minimal insurance.  You will guide the parents on how to achieve the minimum state health requirements for both the child and themselves, i.e. vaccination, good nutrition, a safe and healthy home environment etc.

You are to use MAP-IT: Mobilize, Assess, Plan, Implement, and Track to respond to this SLP.

Steps in MAP-IT are:

1)  Mobilize individuals and organizations that care about the health of your community into a coalition.

2)  Assess the areas of greatest need in your community, [in this case, assisting new immigrants to access health care], as well as the resources and other strengths that you can tap into in order to address those areas.

3)  Plan your approach: start with a vision of where you want to be as a community, then add strategies and action steps to help you achieve that vision: you can be a health educator, a health care provider, a nutritionist, a social worker etc.. 

4)  Implement your plan using concrete action steps that can be monitored and will make a difference.

5)  Track your progress over time.

Using this MAP-IT approach, a step-by-step, structured plan can be devised to tailor one’s community needs. 

The first step in building a healthier community is to mobilize key individuals and organizations to form a community-wide coalition. Most communities already have health departments and other governmental agencies that are responsible for public health services. Many communities also have coalitions of key individuals and organizations that are organized to address specific issues, for example, block associations or neighborhood watch groups. These groups often represent diverse interests and resources for addressing issues that are vital to building and maintaining the health and stability of the entire community. A coalition will often, of course, work with the health department and other health organizations in the community. However, it can also help mobilize a wider range of other resources to address health issues.

You are to complete the following tasks for each module as highlighted.

Module 1:  You have completed this module SLP which was to:

        a)  Select which family you for which you will use the MAP-IT Approach for the SLP.  

        b)  Discuss how you will Mobilize individuals and organizations that care about the health of your community into a coalition. Identify the individuals and organizations which will form the basis of your coalition.  Discuss how each member of your coalition can assist each member of your target family.

COURSE DESCRIPTION and SUMMARY:

This course provides an overview of the fundamental theories and practice of working with different types of group, including therapeutic, task and action groups. Students will explore issues related to group development and process. Special attention is given to issues related to leadership, power, teamwork and collective ways of working together. Through a combination of reflective, didactic and experiential learning activities, students will be encouraged to explore their own interpersonal behaviours and those of others in a group context. The activities focus on interpersonal communication skills, feedback and self disclosure skills and intervention techniques. Leadership, therapeutic boundaries and ethical guidelines for group work practice are also examined.

The main ethical dilemma presented in this case of a nurse who is a whistle blower is that she is obliged to report a doctor who she believes is guilty of malpractice, yet she knows that her action will have far reaching consequences such as losing her job, especially because she used her work computer. Confronted with this situation she makes up her mind to do what she feels is the right thing. She reports to the administrator, who does little or nothing about it. It is only then that she writes an anonymous letter to the Texas medical Board. Her action has dire consequences: facing felony charges in a court of law. She has a duty to protect patients from ill medical practices such as performing skin graft operations in the emergency room without surgical privileges and using unconventional remedies to treat patients. The dilemma she is presented with is whether she should fight for the patients’ rights or fight the rogue doctor who has powerful friends in places of power.
Question 2: Avenues at the disposal of nurses to deal with dilemmas at the work place
A few options are available for nurses to deal with ethical dilemmas. Though the administration in this case showed laxity in dealing with the matter, it is a good option for nurses to report such matters to the administrators. Secondly, medical boards are effective in dealing with ethical issues as that is what they are mandated to do. Reporting to such authorities will help to deal with these issues in one way or another. Filing anonymous complaints is another option. Turning to the state and national nurses association is very helpful. In this case these institutions have raised funds to defend the nurse.
