Cloud-based system refers to database programs, services or resources that are made available to users on demand via the Internet from a cloud computing provider’s server.

 

A cloud-based EHR system refers to an electronic health record system where data is stored on external servers and can be accessed via the web. The only requirement is a computer with an Internet connection. (North Carolina Institute of Medicine and The Duke Endowment, 2016).

 

The system implemented on the premise is also called client- server system. Data is stored in local servers in a designated data center. This system requires an internal server, a hardware and a software to be installed in the physician’s office.

 

The difference between cloud-based and client-server system stands first on the “Virtuality” of the system. The cloud-based system uses the web to store data using an external server whereas the client -base system uses a physical hardware in the location for data storage. Since the cloud-based system uses the web, it is easily accessible anywhere internet is present which is different for the client-based server.

 

Digital health record while being indispensable nowadays present many benefits and drawbacks.

 

From using the cloud -based system we can cite two benefits.

 

1-It increases interoperability- A cloud-based EHR system can be accessed from anywhere which means this EHR can communicate with other EHRs, laboratories, pharmacies and facilitate record sharing instantly with any authorized provider. Test results and patient encounters information are directly submitted to the EHR to update the digital patient record.

 

2-With cloud-based system, large companies provide more powerful IT service than in-house IT. It doesn’t require experts to install, configure, test, run, secure and update hardware and software, all of that is done internally in the cloud services rendered by the cloud provider which will ultimately decrease the cost associated with the EHR.

 

While the cloud-based system present so many benefits we cannot forget to mention major drawbacks as well.

 

  1. Data security is the first concern that most providers are worried about. Adhering to HIPAA requirements is the clinician first responsibility. Protecting patient’s health information, controlling the accessibility to data usually influences a provider on choosing a client-server or cloud-based system. Cloud-based servers are more liable to malware, virus, and hacking attempts.
  2. The providers have “lower level of data access and control compared to an on-site EHR system”. (Office of National Coordinator for Health Information Technology, 2019). Because it is web-based, practice cannot work if there is no internet connection hence limiting the access to data. It is particularly important to rural areas where internet is not as accessible as urban areas.

You are the regional sales manager for International Herb Express (IHE) which is a large, US-based spice & herbs supplier. With technological advancements today, industries are seeking to maximize profits, cut cost, be it administrative or infrastructure. This desire to stay in business, offer the best services or most competitive has lead even the health industry to seek better ways to offering patients the most effective and efficient care at a lower cost if possible. Most of this is thanks to how we store and consume data today.

 

Sharing EHR information today has proven to have improved accessibility to people receiving medical services and or information at remote site. And cloud computing paradigm is one of the popular healthIT infrastructures for facilitating EHR sharing and EHR integration (Chen, et. al, n.d, and p.3375).

 

Cloud-based systems then are IT infrastructure systems that enable data to be virtually stored in another location that is connected to the on premise network. This information as such can be accessed at any location and time. Just like the name cloud, the storage is virtual, located out of the organization and managed either by the same organization or a third party. On the premise systems on the other hand are IT infrastructures and applications that are located within the organization and you must be within the organization and the network to access data.

 

The cloud-computing infrastructures deliver information technology as services, by enabling the renting of software, computing power and storage. It provides an attractive IT platform to cut down the cost of EHR systems in terms of both ownership and IT maintenance burdens for many medical practices. It’s important cornerstones to streamline healthcare whether it is for maintaining health records, monitoring of patients, managing diseases and cares more efficiently and effectively, or collaboration with peers and analysis of data (Chen, et. al, n.d, p.3376).

The benefits of cloud-based computing are numerous.

 

a) Using cloud-based system can drastically reduce cost and enhance patient outcome. Deploying to cloud implies the organization will cut down the IT infrastructure cost and personnel as well as maintenance cost.

 

b) Increasing patient outcome, because it gives the physician the flexibility to access stored patient data at any time and place and can collaborate with other physicians regarding patient care.

 

3) Identify two risks or drawbacks of using cloud-based systems

 

a) Using cloud solution could increase liability of the cloud vendor not being compliant with HIPAA regulations, which requires data storage companies that have access to protected health information (digital or hard copy) to be directly liable in case of security breaches (Shwayri, 2014, p.43).

 

b) Downtime can pose serious risk in case of emergencies as the systems rely solely on the internet.
