The purpose of this assignment is to practice the skills you have learned and demonstrate your ability to assess the thorax, lungs, and cardiovascular system. For this assignment you will need to videotape yourself examining the thorax, lungs and cardiovascular system on a volunteer of your choosing. This should take you between 30-45 minutes. When you are done you will document your findings. You will need to submit both your video and your documentation. Below are the required pieces in your assessment.

Thorax:

  • Assess the patients for any signs of respiratory difficulty
  • Inspect the shape and movement of the chest
  • Asses for chest expansion
  • Palpate for tactile fremitus posteriorly
  • Percuss the posterior chest
  • Percuss for diaphragmatic excursion
  • Auscultate posterior breath sounds
  • Auscultate for egophony, bronchophony and pectoriloquy
  • Examine the anterior chest
  • Observe for any abnormalities (size, shape, bruising)
  • Auscultate for tactile fremitus anteriorly
  • Percuss the anterior chest
  • Auscultate anterior breath sounds

CV:

  • Assess JVP
  • Assess carotid pulses
  • Assess the brachial artery
  • Inspect and locate the PMI
  • Palpate for thrills/bruits

Overview: As a community of practice, your task for this week is to  collaborate with professionals across the health care system. With your  community of practice, in the Discussion, find a gap in care or social  determinant that often results in poor health care outcomes. You will  begin to take the lead in advocating for and collaborating with others  to improve the health care outcomes for populations at risk.

Practicum Discussion: During this week, you will identify a  population at risk in your community. This population will be the basis  for your Practicum Discussions and your individual presentations over  the next 6 weeks, as well as the focus of your final PowerPoint  presentation in Week 6. To review, a population is a group of  individuals who share a common environmental or personal characteristic,  such as obese individuals who are at risk for diabetes or  cardiovascular disease (populations at risk) or those individuals who  are otherwise healthy and could stay healthy if they do not develop  risky behaviors (populations of interest). An example of this is  teenagers who dont yet smoke but might consider it due to peer pressure  (Stanhope & Lancaster, 2016). Some of the topics you might consider  are vaccination compliance, obesity rates among children and adults,  teenage pregnancy, or infectious diseases such as Norwalk virus, genital  warts, or sexually transmitted diseases/infections. You might look also  at emerging public health problems such as Chagas or the Zika virus.  Some of the places you might consider looking for information to  substantiate and support your ideas about populations at risk in your  communities are your local health department, the CDC, and the many  evidence-based websites that the CDC supports, such as the CDC Wonder  (http://wonder.cdc.gov/). You may also review the work of other  community groups that focus on improving health care outcomes for your  community. You should begin to support your selection of population and  ideas about their health care problems through the use of health data  and scholarly literature.

Please address the following points in your Practicum Discussion:

  • Briefly describe your community and then describe your practice setting.
  • What are the determinants of health in your community?  (https://www.healthypeople.gov/2020/about/foundation-health-measures/Determinants-of-Health)
  • What are the most prevalent health problems in your community or in your practice?
  • Choose a population at risk and describe the health problem specific to that population. Remember t

Theories of successful aging explain factors that support individuals as they grow old, contributing to their ability to function. Increasing your understanding of factors that support successful aging improves your ability to address the needs of elderly clients and their families.

To prepare for this Discussion, review this week’s media. In addition, select a theory of successful aging to apply to Sara’s case.

Parker Family Episode 2 Program Transcript PARKER: Ever since my husband died, there’s been no one to talk to. It’s just, really, no one. And when Stephanie is home, I just feel so alone. FEMALE SPEAKER: What about the day center you go to? Isn’t that helping? PARKER: I don’t like it. What makes me really feel good, though, is when I go shopping, buying things. And my kittens. I love my cats. Oh, have you seen them? I have pictures. Just take a look. Look! These are so cute. My babies. FEMALE SPEAKER: Yes, they’re very cute. And wow, you have a lot of them.  PARKER: Oh, well, it’s their home, too, not just Princess Stephanie’s.  FEMALE SPEAKER: The day program you’re attending, are you seeing a  psychiatrist there?  PARKER: Yes. Dr. Lewin.  FEMALE SPEAKER: May I ask how that’s going?  PARKER: He says that I’m depressed.  FEMALE SPEAKER: In the pictures you showed me, you just talked about the  cats, but I also saw all the things you keep around you, the hoarding. I understand how depressed you been since your husband passed away. How alone you’ve felt. [SIGH] But I would like us to try and set up a plan to begin to address the hoarding. It’s very clear that that’s one of the big issues that’s affecting your relationship with Stephanie and your life together. Can we try that? PARKER: I don’t like it when we fight. She’s still my baby, too. Yes, I want to try.

By Day 3

Post a Discussion in which you:

  • Explain key life events that have influenced Sara’s relationships. Be sure to substantiate what makes them key in your perspective.
  • Explain how you, as Sara’s social worker, might apply a theory of successful aging to her case. Be sure to provide support for your strategy.

Select a global health issue affecting the international health community. Briefly describe the global health issue and its impact on the larger public health care systems (i.e., continents, regions, countries, states, and health departments). Discuss how health care delivery systems work collaboratively to address global health concerns and some of the stakeholders that work on these issues.

This week you begin to lay the foundation for your academic and professional success. Your efforts begin with a vision, which includes your own definition of success. Your vision may vary from those of your colleagues, but this does not mean you have to take these first steps alone.

Walden University and the School of Nursing (SON) also have a vision and mission, which include helping you to make your own vision a reality. Members of your new academic community, such as faculty, support teams, and fellow students, can also be helpful. Current practitioners and other members of the professional community can also help you to clarify your vision.

This weeks Discussion asks you to think about how the Walden mission and vision and the School of Nursing (SON) mission and vision apply to your professional and academic goals. You will also begin to identify individuals and teams that can help you along the way, as you begin to design the blueprintyour Academic Success and Professional Development Planthat will guide you toward your own vision for academic and professional success.

To Prepare:

  • Review the Walden and the School of Nursing (SON) mission and vision statements, Waldens goals and University Outcomes, and the MSN Program Learning Outcomes presented in this week’s Learning Resources.
  • Reflect on your professional and academic goals as they relate to your program/specialization.
  • Consider how the information in these resources fit with your own goals and to your becoming a scholar-practitioner.
  • Also consider academic and professional individuals and teams that you may collaborate with in support of your efforts as a student at the University and as a professional within your organization and career.

Write a paper (2,000-2,500 words) in which you apply the concepts of epidemiology and nursing research to a communicable disease. Refer to “Communicable Disease Chain,” “Chain of Infection,” and the CDC website for assistance when completing this assignment.

Is this a reportable disease? If so, provide details about reporting time, whom to report to, etc.
  2. Describe the social determinants of health and explain how those factors contribute to the development of this disease.
  3. Discuss the epidemiologic triangle as it relates to the communicable disease you have selected. Include the host factors, agent factors (presence or absence), and environmental factors. Are there any special considerations or notifications for the community, schools, or general population?
  4. Explain the role of the community health nurse (case finding, reporting, data collection, data analysis, and follow-up) and why demographic data are necessary to the health of the community.
  5. Identify at least one national agency or organization that addresses the communicable disease chosen and describe how the organizations contribute to resolving or reducing the impact of disease.
  6. Discuss a global implication of the disease. How is this addressed in other countries or cultures? Is this disease endemic to a particular area? Provide an example.

A minimum of three peer-reviewed or professional references is required.
