Develop a 3-4 page preliminary care coordination plan for a hypothetical individual in your community. Identify and list available community resources for a safe and effective continuum of care.

NOTE: You are required to complete this assessment before Assessment 4.

The first step in any effective project or clinical patient encounter is planning. This assessment provides an opportunity for you to strengthen your understanding of how to plan and negotiate the coordination of care for an individual in your community as you consider the patient’s unique needs; the ethical, cultural, and physiological factors that affect care; and the critical resources available in your community that are the foundation of a safe plan for the continuum of care.

As you begin to prepare this assessment, you are encouraged to complete the Care Coordination Planning activity. Completion of this will provide useful practice, particularly for those of you who do not have care coordination experience in community settings. The information gained from completing this activity will help you succeed with the assessment. Completing formatives is also a way to demonstrate engagement.

Demonstration of Proficiency
By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

Competency 1: Adapt care based on patient-centered and person-focused factors.
Analyze a health concern and the associated best practices for health improvement.
Competency 2: Collaborate with patients and family to achieve desired outcomes.
Establish mutually agreed-upon health goals for a care coordination plan, in collaboration with the patient.
Competency 3: Create a satisfying patient experience.
Identify available community resources for a safe and effective continuum of care.
Competency 6: Apply professional, scholarly communication strategies to lead patient-centered care.
Write clearly and concisely in a logically coherent and appropriate form and style.
Preparation
Imagine that you are a staff nurse in a community care center. Your facility has always had a dedicated case management staff that coordinated the patient plan of care, but recently, there were budget cuts and the case management staff has been relocated to the inpatient setting. Care coordination is essential to the success of effectively managing patients in the community setting, so you have been asked by your nurse manager to take on the role of care coordination. You are a bit unsure of the process, but you know you will do a good job because, as a nurse, you are familiar with difficult tasks. As you take on this expanded role, you will need to plan effectively in addressing the specific health concerns of community residents.

As you assume your expanded care coordination role, you have been tasked with addressing the specific health concerns of a particular individual within the community. You decide to prepare a preliminary care coordination plan and proceed by identifying the patient’s three priorities for health and by investigating the resources available in your community for a safe and effective continuum of care.

To prepare for this assessment, you may wish to:

Review the assessment instructions and scoring guide to ensure that you understand the work you will be asked to complete.
Allow plenty of time to plan your patient clinical encounter.
Be sure that you have a hypothetical patient in mind.
Note: Remember that you can submit all, or a portion of, your draft plan to Smarthinking Tutoring for feedback, before you submit the final version for this assessment. If you plan on using this free service, be mindful of the turnaround time of 2448 hours for receiving feedback.

Instructions
Note: You are required to complete this assessment before Assessment 4.

Develop the Preliminary Care Coordination Plan
Complete the following:

Identify a health concern as the focus of your care coordination plan. Possible health concerns may include, but are not limited to:
Stroke.
Heart disease (high blood pressure, stroke, or heart failure).
Home safety.
Pulmonary disease (COPD or fibrotic lung disease).
Orthopedic concerns (hip replacement or knee replacement).
Cognitive impairment (Alzheimer’s disease or dementia).
Pain management.
Mental health.
Trauma.
Identify available community resources for a safe and effective continuum of care.
Document Format and Length
You can use the linked templates as a guide for the needs of your hypothetical patient who has a selected health care problem.

For your care coordination plan, you may use the Care Coordination Plan Template [DOCX], choose a format used in your own organization, or choose a format you are familiar with that adequately serves your needs for this assessment.

Your preliminary plan should be 34 pages in length. In a separate section of the plan, identify the hypothetical person you have chosen to work with.
Document the community resources you have identified using the Community Resources Template [DOCX].
Supporting Evidence
Cite at least two credible sources from peer-reviewed journals or professional industry publications that support your preliminary plan.

Grading Requirements
The requirements, outlined below, correspond to the grading criteria in the Preliminary Care Coordination Plan Scoring Guide, so be sure to address each point. Read the performance-level descriptions for each criterion to see how your work will be assessed.

Analyze your selected health concern and the associated best practices for health improvement.
Cite supporting evidence for best practices.
Consider underlying assumptions and points of uncertainty in your analysis.
Identify a hypothetical individual who would benefit from a care coordination plan.
Document goals for the care coordination plan.
Identify available community resources for a safe and effective continuum of care.
Write clearly and concisely in a logically coherent and appropriate form and style.
Write with a specific purpose with your patient in mind.

BELOW IS THE TOPIC-DISCLOSURE: I JUST NEED TO REPLY TO THE STUDENTS POST
Disclosure is important to think about these days – we want to be open and friendly at work, but being too open can lead a person into trouble. The four panes of the Johari Window, help explain the process of self-disclosure and consider our own levels of self-awareness in the communication process.

Hello Everyone,

The Johari Window is a great example of what we face in the workplace and in our everyday lives. Open- The open area in the Johari Window represents your “Public” or “Awareness” area. In this area, it contains information you know and others know about you. When it comes to working with others, it is important to keep an open but not too personal relationship with your co-workers. You want to be open enough to where people don’t look at you as the “Weird Guy” in the office space. Having casual conversations such as “How are you?, How is your day going? That presentation you did today was good, can I hear more? will cover this area of openness.
The Blind area consists of information others know about you but you’re not aware of it yourself. Being blind in certain situations can affect you in a negative way. You may think you’re coming off as supportive but your communication style to others may be directive. Taking feedback and listening to the opinions of the ones around you can weaken that blind area and make you self-aware.
The Hidden area contains information about you that you know but others do not. I feel keeping something hidden from your work peers is healthy. You do not want to give too much information about yourself, especially personal information. Sharing is ok but I feel like even the most open person keeps at least one or two things to themselves. The Unknown area is made up of things unknown to you and others. A lot of us have an unknown area and many still question where their life will end up. People not aware of their capabilities or who lack motivation suffer from the “Unknown”. You may pass up on a job offer because you’re unaware that you may be the best qualified for the job.

INTERNSHIP: GLOBALIZATION: DISCUSSION BOARD REPLY TO STUDENT
AT LEAST ONE REFERENCE APA STYLE
Good Morning Professor Durish and Professor Arszulowica and Class,
Now here we are Week 8. Time is flying. I hope everyone is being safe and having a good weekend.
1. What is your view of Globalization? Don’t just offer the pros and cons, which are important. I want your own viewpoints regarding this topic as well.
– In this week’s discussion, we discuss what Globalization is. The first thing that comes to mind when I think of Globalization is the economy. The reason I say the economy because everyone in this world always talks economy and there are always debates on the economy itself. Every single news channel there is always talking about how the economy could be raised and how it is getting hit. Like right now, the economy is down due to the coronavirus and always companies/businesses/banks/airlines and cruises are taking the biggest hit ever. I really see Globalization as a conflict to the world to be honest, because I personally feel that Globalization has a lot to deal with cultural diffusion. When I say cultural diffusion I feel like Globalization is one of the major things to cultural diffusion because this world always has conflicts with every country and its market itself I feel like. According to Stanford, “Globalization is a politically-contested phenomenon about which there are significant disagreements and struggles, with a growing number of nationalist and populist movements and leaders worldwide (including Turkeys Recep Erdoan, Polands Jaroslaw Kacyzki, Hungarys Viktor Orbn, and US President Donald Trump) pushing back against what they view as its unappealing features”(Scheuerman, 2018). As a result, Globalization is known to have struggles in the economy and many disagreements movements throughout countries. There are many movements through certain countries when doing an exchange in marketing and worldwide itself. When talking worldwide it normally talks about Globalization as itself and how the world changes in an economy and a business itself.
2. How do you see your company in the global market? Are they global? What is the ethnic makeup of associates? Where do the supplies and office equipment originate? Where are they shipped? To whom are services rendered?
– Great question Professor Durish! After reviewing this week’s lecture and required resources I see that the Global market has many different ways of being handled and how it could be controlled. When discussing the Global Market in Northfield Bank where I am currently doing my internship I must say I could see Northfield going far because they have recently brought out another bank and they are starting to do Zelle where you could send someone money if they need it. Northfield Bank is not global right now, only serving New York and New Jersey at the moment. All the ethnic of associates and supplies and office equipment are all located in Woodbridge, NJ where headquarters is and that is where many meetings get taken place. Everything gets shipped to Woodbridge, NJ, and that where things go in and out of. However, yes there could be many effects on the banking floor due to Globalization because of economy and marketing. According to the effects of the workforce, “The wave of economic liberalization that has underpinned this globalization of the world economy reflects a radical change in attitudes towards economic policy. In democratic societies this clearly represents a voluntary shift in political attitudes. But whether democratic or not, ruling regimes across the world, with only a few exceptions, have embraced the view that freer trade, investment and financial flows will be the most effective means for ensuring material prosperity”(Scheuerman, 2018). This website will discuss how Globalization could have an effect on the workplace if there is a problem that cannot be resolved. For example, there could be financial and investment issues in banking or with any company due to the effects and a possible political change which is very true. Banking, however, does deal with certain issues like this and it could take a while to recover and get fixed due to global marketing.

The Final Project Proposal is a written submission in which students identify and discuss the issue(s) surrounding their chosen themes/topics and propose a program, initiative, or activity that will address those issues. For example, a student from a previous semester chose the theme of Climate Change and Natural Disasters for his project package. For the Final Project Product Proposal, he discussed how climate change affects agriculture in South and Central America. Within the proposal, he identified the issue and causes, prior attempts and deficiencies in addressing the issue, and proposed the solution of enhancing global awareness through art (making information pertaining to climate change more appealing and interesting for a targeted audience).

Potential theme/topics/global health problems include, but will not necessarily be limited to:

COVID-19 (highly recommended)
Zika
HIV/AIDS
Obesity & Heart Disease
Food/Water Insecurity, Hunger, & Poverty
Medical Tourism
Climate Change & Natural Disasters
Abortion & Family Planning
Universal Healthcare vs. Private Sector Insurance-based Payers
Health & Wellbeing for Nations at War/Unrest
Vaccinations, Genetic Testing, Autism & Developmental Disabilities
Global Health Security & Preparedness for Aggressive Communicable Diseases
LGBT Community Health
Cancer
Euthanasia / Death & Dying
White, Skinny/Thin, Heterosexual, United Statesian Privilege

Use a standard essay format for responses to all questions (i.e., an introduction, middle
paragraphs and conclusion).

Canada
The Canadian system for health care delivery can be described as a mixed system because it is possible to have completely private health care with the patient paying the provider. However, most Canadian health care is provided through the single-payer model. Each province in Canada has the constitutional option to opt out of this system, although none have to date. All physician, hospital, pharmaceutical, and clinic fees are set and paid for by the government. A health care provider cannot charge more than the government fee, even if the patient is not covered by the public system or is from another country, unless the provider opts out of the system altogether. Other health care expenses such as dentistry and optometry are wholly private-sector operations. Each person who enrolls in the program is given a health care card, much like the Medicare card in the United States. All people have the same plan (http://www.hc-sc.gc.ca).

In recent years, American citizens have increasingly gone to Canada to purchase prescription drugs and be treated for certain illnesses. U.S. citizens take advantage of the high quality and lower cost of Canadian health care. Canadians also come to the United States for elective surgeries such as hip and heart. Canadians travel to the United States for health care for specialized procedures not available in Canada and in some cases to avoid long waiting times.

History
The early French settlers founded Catholic hospitals in the 1800s. Health care in Canada was private until 1946. At that time, the province of Saskatchewan introduced universal health care coverage. Over the next 10 years, the remaining nine provinces introduced similar coverage. In 1957, the Hospital and Diagnostic Services Act was passed through which the federal government paid 50 percent of costs of the health care programs in all 10 provinces. In 1966, the Medical Care Act was passed that instituted the universal coverage countrywide. The Canada Health Act was passed in 1984 that prohibited extra user fees and extra physician billing fees. The Canadian system, which blends universal health coverage with private-sector fee-for-service, is often seen as a model for U.S. health care reform in current and future discussions.

Delivery
The majority of health care in Canada (75%) is funded publicly and delivered privately. The private-sector spending is about 30 percent of the total, primarily for services not covered by Canadian Medicare such as dentistry, optometry, some prescription drugs, cosmetic surgery, and for, in some provinces, priority access to health care services. Canadians may also purchase supplementary private health insurance for these services; about 65 percent of the population has this coverage.

The actual delivery of services is the responsibility of the provinces and territories. Although privately funded, most of the services are provided by private enterprises. Most physicians do not receive a salary, but are paid on a fee-for-service basis. They in turn pay their staff, much like private practice physicians in the United States. The role of the federal government is to:

Provide the funds to deliver the services to the provinces

Set and administer the principles of the national health care system

Deliver services to specific special groupsveterans, First Nation Peoples, and the Inuit people

Provide public health, health protection programs, and health care research

Both Canadian citizens and health care providers report high approval ratings for this system (Health Canada http://www.hc-sc.gc.ca).

Statistics
In 2012, Canada spent 9.8 percent of GDP on health care, or U.S. $4,045 per capita. Of that, approximately 70 percent was government expenditure, according to the World Health Organization (WHO) (Canada http://www.who.int/countries/can).

All students review the healthcare data breaches details found here:

https://healthitsecurity.com/news/the-10-biggest-healthcare-data-breaches-of-2019-so-far

choose any breach numbered 6-10 on the list.

The submission should include these components:

Introduction/Synopsis
This should summarize key details of the events that occurred. Yes, students must include information beyond what is provided in the above link and note Wikipedia is not a reliable source (but is a good starting point to gather general information). The grader will be looking for additional references beyond the course textbook and the provided link.
Using the course textbook, discuss one major concept that you feel the selected organization did not master which eventually lead to the breach. Be sure to substantiate your response with additional external sources (if necessary).
